Registration Forms for down loading

There will be four short forms to down load.  Before any child will be accepted as a camper each of these forms will need to be filled out, signed by a parent or guardian and given to the registrar either by mail or in person.  Each form will be a separate down load.

The first form will be the Registration and Medical Information form.

The second form helps us be sure the camper goes home with the right person at the end of the event. 

The third form you will only fill out on registration day.

The fourth form deals with the camper's medical privacy.

These forms are to be mailed in or delievered in person to the camp.  This is not for on line registration.
